United Healthcare Oxford is a subsidiary of United Healthcare Incorporated. Oxford was formed in 1984 and merged in 2004 into United, the largest healthcare corporation in the United States and the 6th largest company on the Fortune 500.
Oxford Health Plans serve the New York, New Jersey and Connecticut areas but is based out of Minnetonka, Minnesota. Available plans provide traditional health care benefits to employers and individuals. Oxford is a for-profit company with revenues of 84 Billion dollars in 2016. Recently, UnitedHealth Group has paid hundreds of millions of dollars in settlements to the American Medical Association and the State of New York related to the company’s efforts to defraud its customers by manipulating the usual and customary rates paid by insurers. In 2017 the department of justice joined another case where a suit alleges that a United Healthgroup subsidiary defrauded the United States government out of “likely billions- of dollars.”
Oxford frequently changes its clinical guidelines on what will be covered for it’s patients. It lumps “rhinoplasty and other nasal surgeries” together in a 6 page directive last changed July 1, 2017. Where insurance is renewed annually, it is not uncommon for Oxford to revise its policies twice or even three times per year. The insurance also has a disclaimer that: “The inclusion of a code does not imply any right to reimbursement or guarantee claim payment.”
